The objective of this article is to describe a clinical case of oral rehabilitation of a 33-year-old female patient, with symptoms of inflammation and pain in element 24, due to failure in previous endodontic treatment. The image examination revealed root perforations, leakage of obturator material, periapical lesion, and loss of the buccal bone wall. In order to preserve as much bone structure as possible, element 24 was removed in a minimally traumatic way, starting with orthodontic apparatus 72 hours before extraction surgery. After extraction, guided bone regeneration therapy (GBR) was performed with a healing period of 8 months. Then, an implant was installed for subsequent rehabilitation with a single prosthesis.
