Appropriate Planning, thorough Examinations and Decent Implementations for a Predictable Prognosis in the Management of a Severely Worn Dentition: A Case Report 适当的计划,彻底的检查和适当的实施,以获得可预测的预后在严重磨损的牙列管理:一个案例报告

The management of patients with severely worn dentition is challenging due to the loss of occlusal vertical dimension and tooth structure creating an uneven plane of occlusion. This case report describes the importance of every step of the conventional and improvised methods in treating tooth wear patients. Stages from the initial work-up of tooth wear assessment, substantial surgical crown lengthening, the controlled method of increasing vertical dimension, the precise method of crown preparations, advanced impression techniques till the cementation procedure of final restorations. The whole treatment was in a reorganized approach such that the new inter-cuspal position (ICP) coincided with the retruded axial position (RAP). When restoring worn dentition, clinician should always have a proper planning, decent implementation for each stages thus guarantee excellent performances. However maintenance and recall visits are the main keys to long term success.

Hyperglycaemia Accelerates Local Cellular Inflammaging Senescence and Alters the Periodontal Tissues Environment during Periodontitis 在牙周炎期间,高血糖加速局部细胞炎性衰老并改变牙周组织环境

Diabetes mellitus is a group of metabolic diseases which have several aetiologies that involve millions of people around the world. Hyperglycaemia, the hallmark of diabetes mellitus, causes detrimental complications such as nephropathy, neuropathy, retinopathy, and cardiovascular disease in both Type 1 and 2 diabetic patients. Inflammaging is recently linked with the development of diabetic complications. Local cellular senescence and its senescence-associated secretory phenotype (SASP) are the main contributors to inflammaging which can be triggered and accelerated by high glucose level. Regarding the oral cavity, hyperglycaemia, provokes the severity of inflammation and destruction in the tooth supporting structures (periodontium) and increases the susceptibility to periodontitis and eventually tooth loss. This paper provides insights into the impact of hyperglycaemia in causing cellular senescence of teeth supporting tissues and escalating periodontal tissue deterioration.
