Dental Implant Impressions: The First Impression Is The Last Impression: Short Communication 植牙印象:第一印象就是最后印象:简短的交流

Dental implant is defined as a prosthetic device made of alloplastic material(s) implanted into the oral tissues beneath the mucosal and/or periosteal layer and on or within the bone to provide retention and support for a fixed or removable dental prosthesis; a substance that is placed into and/or on the jaw bone to support a fixed or removable dental prosthesis [Glossary of Prosthodontic Terms (GPT 9)]. Dental implants have been swiftly gaining popularity as a treatment modality of choice in prosthodontics i.e. both fixed and removable. Impression is defined as negative likeness or copy in reverse of the surface of an object; an imprint of the teeth and adjacent structures for use in dentistry (GPT 9). Impressions in implantology is a crucial step and involves a meticulous workflow in achieving the passive fit for the long-term success of the final implant prosthesis. This involves accurately relating and recording the exact replica of the implant analogue or the implant abutment to the other structures in the oral cavity. A plethora of factors determine the accuracy of the implant impressions namely the choice of impression tray, impression material, number of implants, depth, position and angulation of implants and most importantly the technique encountered for recording the impression. This short communication briefly portrays the armamentarium and components used along with the selection of a suitable impression tray, impression material and impression technique devised for the precise impression of the implant for the ultimate success of the final prosthesis.

Twin Mix Inferior Alveolar Nerve Anaesthesia for Transalveolar Extraction of Mandibular Third Molar Surgery: Review of Current Evidence 双混合下牙槽神经麻醉用于下颌第三磨牙经牙槽拔牙手术:现有证据的回顾

Utmost recurrent surgical process executed in oral & maxillofacial surgery is transalveolar extraction (TAE) of mandibular impacted 3rd molar which consequences to surgical trauma to surrounding soft & hard tissues during the process of tooth removal. Post-operative sequalae like pain, facial swelling & sometimes trismus follow TAE. Dexamethasone is commonly used steroid post-operatively and studies have revealed clinical aids in refining patients’ quality of life (QoL). To improve the QoL after TAE of third molars, use of twin-mix anaesthesia is proposed, where a combination of local anaesthetic solution and steroid is administered as a single injection, thus, negating the requirement of additional route of administration for dexamethasone. The advantages for this technique are, solitary method for dual drug delivery, reduced sensation of sting while injecting owing to altered pH of solution, short latency period, prolonged duration of anaesthesia & amended QoL post operatively. Twin Mix inferior alveolar nerve anaesthesia is found beneficial by reducing patient discomfort and enhancing patient compliance for surgeries of the mandible.

Zirconia Over Titanium Implants: The Evidences are not Enough 氧化锆优于钛植入物:证据不足

Implant therapy is a widely used treatment modality for completely and partially edentulous patient. It gives excellent long term results and has made practice of dental implantology astonishingly widespread. Titanium dental implants have proved to be successful means of prosthetic rehabilitation for more than six decades. Recently, ceramics have been proposed as an alternative to titanium. Zirconia implants with better aesthetics, mechanical and biological properties are showing a promising future in dental implantology. This narrative review analyses the evidences to compare titanium and zirconium implant in a systemic manner. The paper includes the mechanical, biological and clinical consideration involving both implant materials.

Minimally Invasive Technique for Removal of a Mandibular Radicular Cyst : A Case Report 微创技术去除下颌骨根状囊肿1例报告

The removal of an odontogenic jaw cyst usually requires at least elevation of the mucoperiosteal flap for adequate access to the cystic lesion and its complete enucleation. In this report, we present a simple intra-socket flapless cystic removal technique for a radicular cystic lesion related to the mandibular first molar. This technique produces an optimal outcome and gives the patient the most comfortable surgical experience.

A removable appliance is a device that modifies mandibular posture and transmits the resultant forces created by muscles and soft tissues to underlying and surrounding anatomical structures in a controlled manner. The resulting variation of the neuromuscular environment thus produces the required tooth movement along with the needed advancements in growing patterns. The necessity and requirement of early treatment is to modify the existing and developing malocclusions and muscular derangements before the attainment of growth completion of permanent dentition. Frankel Function Regulator (FR) is a device which functions on the principle of functional orthopedics in unity with muscle gymnastics (muscle exercises) and thereby results in morphological changes in both the jaws hence re-establishing the desirable normal occlusion.

QPCR Screening of Pediatric Saliva Samples to Evaluate Effects of Dental Sealants on Cariogenic Bacteria Streptococcus Mutans and Scardovia Wiggsiae QPCR筛选儿童唾液样本评价口腔密封剂对致龋细菌变形链球菌和威氏卡杜氏菌的影响

Dental caries is an alarmingly prevalent multifactorial disease worldwide but is most common as a chronic disease among children and adolescents. Within the oral cavity there are hundreds bacterial species, including specific cariogenic pathogens such as Streptococcus mutans and Scardoviawiggsiae. Although dental caries are highly prevalent, it may also be preventable using methods such as dental sealants or fluoride treatment, which have proven to be highly effective in reducing caries risk – although there is limited evidence on the exact effect of these treatments on S. wiggsiaeinparticular. Due to the lack of conclusive evidence, this study is aimed to evaluate the microbial burden of S. mutans and S. wiggsiae among pediatric saliva samples to analyze the effects dental sealants have on these oral microbiota. Saliva samples derived from pediatric patients pre- and post-sealant were obtained from an existing repository. DNA was extracted and screened using qPCR and primers specific for the cariogenic organisms, S. mutans (SM) and S. wiggsiae (SW). These data demonstrated an overall reduction in SW among Post-sealant samples (11.14 U/uL) compared with Pre-sealant samples (11.68 U/uL), as well as a reduction in the overall number of samples harboring S. wiggsiae (n=8). The Pre-sealant average amount of SM (14.54 U/uL) was slightly increased among the Post-sealant samples (15.67 U/uL) with half of the samples analyzed (n=13) exhibiting slight increases in SM levels, and nearly one-third (n=8) exhibiting slight decreases in SM levels. The data from this study may suggest that dental sealants have an impact on certain cariogenic pathogens in the oral microbiota. Whether or not the impact is positive or not in light of the increase in S. mutans remains an observation that should be further analyzed. More research is required to address the limitations found in this study in order to localize the exact effect on the teeth with dental sealants.

Within the oral cavity there are hundreds bacterial species, including specific cariogenic pathogens such as Streptococcus mutans and Scardoviawiggsiae. Although dental caries are highly prevalent, it may also be preventable using methods such as dental sealants or fluoride treatment, which have proven to be highly effective in reducing caries risk – although there is limited evidence on the exact effect of these treatments on S. wiggsiaeinparticular. Due to the lack of conclusive evidence, this study is aimed to evaluate the microbial burden of S. mutans and S. wiggsiae among pediatric saliva samples to analyze the effects dental sealants have on these oral microbiota. Saliva samples derived from pediatric patients pre- and post-sealant were obtained from an existing repository. DNA was extracted and screened using qPCR and primers specific for the cariogenic organisms, S. mutans (SM) and S. wiggsiae (SW). These data demonstrated an overall reduction in SW among Post-sealant samples (11.14 U/uL) compared with Pre-sealant samples (11.68 U/uL), as well as a reduction in the overall number of samples harboring S. wiggsiae (n=8). The Pre-sealant average amount of SM (14.54 U/uL) was slightly increased among the Post-sealant samples (15.67 U/uL) with half of the samples analyzed (n=13) exhibiting slight increases in SM levels, and nearly one-third (n=8) exhibiting slight decreases in SM levels. The data from this study may suggest that dental sealants have an impact on certain cariogenic pathogens in the oral microbiota. Whether or not the impact is positive or not in light of the increase in S. mutans remains an observation that should be further analyzed.
