A study was conducted on 25 cases of tibial fractures (between April 2021 to April 2022) in which RCT tibial nail introducer was used as a nail-positioner-cum-nail-introducer (Group 1) to treat the tibial fractures. The results of this study was retrospectively compared with 25 cases of tibial fractures treated by Dr Thakur Innovative Technique of Suprapatellar Nailing of Tibia (between March 2020 to March 2021) in which infrapatellar-tibial-nail-extractor was used as nail-positioner-cum-introducer (Group 2). It was found that the surgical time and fluoroscopy time was slightly less in Group 1 compared to Group 2. But the nail could be kept in proper position by deroation while hammering was more easily and accurately done in Group 1 compared to Group 2. In both groups, the fixation was done by Dr Thakur Innovative Technique of Suprapatellar Nailing of Tibia and locking was done by free hand technique both proximally as well as distally.

Introduction: Lowback pain is one of the most common chronic pain disorders, with 2-3% incidence of lumbar disk herniations among population. Epidural corticosteroid injections have been used since 1952 for treatment of lumbar radiculopathy with or without discogenic back pain in patient with failed medical and conservative management. Local Steroids such as triamcinolone acetate acts by limiting inflammatory response. Several approaches are available; interlaminar, transforaminal and caudal. This study was conducted to compare the efficacy of triamcinolone acetate injection by interlaminar vs transforaminal approach. AIM: To compare the efficacy of triamcinolone acetate in terms of pain relief between interlaminar approach and transforaminal approach. Methodology: In this study 60 patients with low back pain are randomly allocated to one of the two groups of 30 patients each. In both group 40mg of triamcinolone acetate solution is used. In IL approach, drug is injected blindly in the epidural space, while in TF approach drug is injected under fluoroscopic guidance using dye. Outcome consists of measuring pain relief at 2nd and 4th week after injection using Numerical Scale and Verbal Rating Scale. Data was collected after 15mins, at 2nd week and at 4th week after the injection. RESULTS : The pain relief in NRS and VAS at 2 nd week was more in patient who received triamcinolone acetate via transforaminal approach. Conclusion: Transforaminal is superior to interlaminar, as it gives target specific administration of triamcinolone acetate and is more effective than ILESI comparing the NRS and VAS score in terms of pain relief.

Introduction: Bilateral Spontaneous Quadriceps tendon rupture is a rare occurrence and a few handful cases have been reported in literature worldwide. Case Presentation: A 57-year-old patient with Vitiligo and on oral steroids falls while trying to get up from a chair and complains of weakness in both knees, unable to get up. Patient was found to have osteo-tendinous rupture of Quadriceps tendon in both knees which occurred spontaneously leading to weakness and fall. Conclusion: Prompt Orthopaedic attention is required with such conditions instead of giving priority to neurological review and consult for weakness as results are better in early diagnosis & surgery.

Background: Lateral epicondylitis affects people mainly in middle-aged groups. Despite numerous studies, it is still a poorly understood illness with no clear treatment guidelines. The goal of the study was to compare deep friction massage vs corticosteroid injection for the management of lateral epicondylitis. Methods: To compare outcomes, a randomised controlled trial was conducted. The patients were divided into 2 different groups. The first group was given a corticosteroid injection, while the second was given a deep friction massage. Disabilities of the Arm, Shoulder, and Hand (DASH) scores, visual analogue scale (VAS), and grip strength were measured before and after treatment. Results: The results were expected in 6-12 weeks, with a 6-month follow-up. At the 6-to 12-week follow-up, both groups showed significant improvements in VAS pain score, DASH score, and grip strength. At the 6-month follow-up, all three end measures, VAS pain score, grip strength, and DASH score, indicated a substantial improvement in group 2 deep friction massage. Conclusion: For lateral epicondylitis, deep friction massage is the best line of management and can be utilized in patients for whom various treatment options have failed. This form of treatment gives good functional outcomes without any complications as compared to a corticosteroid injection.

Introduction: Infected tibial non-union has always posed a challenge for orthopaedic surgeons. The primary aim of our study is to analyze the role of Ilizarov circular ring fixation in infected non-union tibia, to assess osteogenesis, infection rates, bony union, and limb lengthening along with functional outcomes and associated complications. Materials and Methods: This observational study was conducted at the Department of Orthopedics in Tertiary Care Centre of western India. A total 20 cases of infected non-union of diaphyseal # of Tibia with chronic discharging sinus of at least 3 months duration were included in our study. All procedures were performed after the patients had signed a written consent and after approval by the institutional ethical committee. Results: In our 20 cases, union was achieved in 19 cases. One patient required bone grafting and one patient had refracture. Union time ranged between 3 to 7 months. Smoking, persistent, infection, patient himself not able to walk, these factors affect the union. Early docking of the non-union site was observed to shorten the union time.
