Hemodynamic index, brain injury marker index, stress response index, awakening condition, propofol dosage, anesthetic effect, and adverse reactions were assessed and recorded.Results: Heart rate (HR), diastolic blood pressure (DBP), and systolic blood pressure (SBP) at T2 and T3 of the two groups were less than those at T1. At T3, the study group’s HR, DBP, and SBP were substantially higher than those of control group (p < 0.05); At 12 and 24 h after operation, brain injury markers and stress response indices in study group were significantly lower compared to control group (p < 0.05), while in comparison to control group (79.55 %), the degree of anesthesia in the study group was higher (95.45 %; p < 0.05). The incidence of adverse reactions in the study group (15.91 %) was lower than in control group (43.18 %; p < 0.05).Conclusion: Remifentanil, when combined with propofol anesthesia, stabilizes hemodynamics, protects against brain injury, and reduces stress reactions in patients undergoing cerebral hemorrhage surgery. The combination is also highly effective and safe. 摘要

目的:探讨脑出血手术患者同时应用瑞芬太尼和异丙酚的麻醉效果及相关指标。方法:将2019年12月至2020年12月在南京市丽水区人民医院住院的脑出血患者88例分为两组,对照组采用芬太尼+异丙酚麻醉,研究组采用瑞芬太尼+异丙酚麻醉,每组44例。评估并记录血流动力学指标、脑损伤标志物指标、应激反应指标、苏醒情况、异丙酚用量、麻醉效果及不良反应。结果:两组患者T2、T3时心率(HR)、舒张压(DBP)、收缩压(SBP)均低于T1时。在T3时,研究组的心率、舒张压和收缩压明显高于对照组(p <0.05);术后12、24 h,研究组脑损伤标志物及应激反应指标均显著低于对照组(p <0.05),而与对照组(79.55%)相比,研究组麻醉程度较高(95.45%;p & lt;0.05)。研究组不良反应发生率(15.91%)低于对照组(43.18%);p & lt;0.05)。结论:瑞芬太尼联用异丙酚麻醉可稳定脑出血手术患者的血流动力学,防止脑损伤,减轻应激反应。这种组合也非常有效和安全。然而,需要在更大规模的临床试验中验证这些发现。
