Several trigger mechanisms can be identified in the pathogenesis of epidural fibrosis formation: interaction of intervertebral cartilage components with surrounding tissues, disruption of vertebral canal integrity, dystrophy or complete absence of epidural fat, and presence of blood in the postoperative area. The pain syndrome associated with epidural fibrosis is persistent and increases after walking and prolonged being upright. There may be hyperaesthesia or hyperpathia in the innervation area of the affected roots and brisk tendon reflexes. Many authors are now trying to predict epidural fibrosis by analyzing immunological parameters and osteoprotegerin levels in blood serums, assessing anthropometric measures of patients, and examining the blood coagulation system. In modern neurosurgical society there is a clear view that epidural fibrosis should be prevented, but there are many prevention techniques.
