In severe cases, surgery may be required.Surgical strategies for C-OPLL can be divided into anterior and posterior approaches, each offering distinct advantages and limitations. Anterior approaches, such as anterior cervical corpectomy and fusion (ACCF), anterior cervical discectomy and fusion (ACDF), anterior floating method and vertebral body sliding osteotomy (VBSO), provide the benefit of direct decompression but are technically demanding and carry a higher risk of complications. In contrast, posterior approaches, including laminectomy (with or without instrumented fusion) and laminoplasty (LAMP), offer indirect decompression but may increase the risk of cervical kyphosis. In recent years, innovative techniques like anterior cervical ossified posterior longitudinal ligament en bloc resection (ACOE), anterior controllable antedisplacement and fusion (ACAF), and minimally invasive endoscopic spine surgery have been developed to reduce complications and enhance surgical outcomes.Selecting the appropriate surgical technique requires a thorough assessment of factors such as the severity of the lesion, cervical alignment, and the surgeon's experience.
