摘要

目的:探讨局部动脉内化疗联合免疫检查点抑制剂治疗不可切除头颈部鳞状细胞癌(HNSCC)的疗效和安全性。方法:回顾性分析我院2023年1 - 12月确诊为不可切除的HNSCC患者8例的临床资料。所有患者均经病理诊断,动脉内化疗联合免疫检查点抑制剂治疗。观察总有效率(ORR)、无进展生存期(PFS)、总生存期(OS)和不良反应。结果:研究队列包括7男1女,中位年龄58.4±13.1岁。5例患者处于IVA期,1例处于IVB期,2例处于IVC期。1例失访,7例资料完整的患者,ORR为85.7%。总体人群的中位PFS和OS没有达到,患者对治疗的耐受性良好,没有明显的不良反应。结论:局部动脉内化疗联合免疫检查点抑制剂是未来不可切除的恶性鳞癌治疗的重要探索方向。

Safety and Efficacy of Intra-Arterial Chemotherapy Combined With Immune Checkpoint Inhibitors in the Treatment of Advanced Head and Neck Squamous Cell Carcinoma

Objective

To investigate the efficacy and safety of local intra-arterial chemotherapy combined with immune checkpoint inhibitors in the treatment of unresectable head and neck squamous cell carcinoma (HNSCC).

Methods

A retrospective analysis was conducted on the clinical data of eight patients diagnosed with unresectable HNSCC at our hospital from January to December 2023. All patients were diagnosed by pathological diagnosis and treated with intra-arterial chemotherapy combined with immune checkpoint inhibitors. The overall response rate (ORR), progression-free survival (PFS), overall survival (OS), and adverse reactions were observed.

Results

The study cohort consisted of seven men and one woman, with a median age of 58.4 ± 13.1 years. Five patients were at stage IVA, one at stage IVB, and two at stage IVC. One patient was lost to follow-up, and among the 7 patients with complete data, the ORR was 85.7%. The median PFS and OS in the overall population were not achieved, and the patient tolerated the treatment well with no significant adverse effects.

Conclusion

Local intra-arterial chemotherapy combined with immune checkpoint inhibitors is an important future exploration direction for the treatment of unresectable HNSCC.
