本文采用定量与定性相结合的方法,对2001-2002年间45例出院后体弱老年患者的病例管理过程进行了研究。定量数据提供了客户主动拨打护士病例管理人员电话的常见原因以及护士病例管理人员对这些电话的干预。定性数据产生了9个主要主题,在此基础上概念化了病例管理的顺序和动态过程模型。另外7项关于成功实施个案管理的基本因素的专题说明是在工作人员和结构因素的双重框架内编制的。
