Despite an excellent track record, complications still exist. The purpose of this article is to introduce a case series of modified and grafted coronectomy (MGC), designed to minimize the drawbacks of SC.</p><p><strong>Materials and methods: </strong>Sixteen patients, with total of 20 teeth, were followed for 6 to 49 months. Amendments to SC were intra-surgical stabilization of the root and the creation of a periodontal \"scaffold.\"</p><p><strong>Results: </strong>MGC showed excellent alveolar bone height and periodontal improvement, with remarkable bone regeneration.
