In three patients the osteotomy was bilateral, totalling 12 cuffs analyzed. The indication for the technique described was deformity and stiffness for over six months, without improvement with the conservative treatment.</p></div><div><h3>Results</h3><p>The mean age of the patients on the day of surgery was five years and eight months. The initial mean wrist mobility was 35°, and the joints presented a mean flexion of 72.5° in a resting position. Osteotomy union occurred in all patients at an average period of 5.7 weeks. The final position of the resting wrist was 12° of flexion and the mean mobility was 26.6°, slightly lower than preoperatively but in a much better position. No serious complications arising from surgery or in the immediate postoperative period were observed.</p></div><div><h3>Conclusions</h3><p>Carpal osteotomy with biplanar dorsal resection wedge was useful and effective in helping to correct the deformities in flexion and ulnar deviation of the wrist, maintaining a reasonable mobility. 目的探讨双面腕楔截骨术治疗先天性关节挛缩合并腕部畸形的疗效。方法回顾性分析2004年1月至2009年12月收治的9例腕关节严重屈曲尺偏畸形患者。他们接受双面背侧楔形切除腕骨切开术,术后进展至少评估48个月。其中3例患者为双侧截骨术,共分析了12个袖口。该技术的适应症是畸形和僵硬超过6个月,保守治疗没有改善。结果手术当日患者平均年龄5岁8个月。初始平均手腕活动度为35°,关节在静止位置平均屈曲72.5°。所有患者截骨愈合的平均时间为5.7周。最终静息腕关节屈曲12°,平均活动度26.6°,略低于术前,但位置好得多。术中及术后未见严重并发症。结论双面背侧楔形截骨术能有效矫正腕关节屈曲、尺偏畸形,保持合理的活动能力。这是一种保留手术,发病率低,避免了畸形的进展和未来的退行性改变。

Biplanar carpal wedge osteotomy in the treatment of the arthrogrypotic patients

Objective

To describe the results of the surgical treatment of patients with congenital arthrogryposis with wrist deformity through biplanar carpal wedge osteotomy.

Method

This study analyzed nine patients through a retrospective evaluation with severe deformity in flexion and ulnar deviation of the wrist in the period between January 2004 and December 2009. They were submitted to carpal osteotomy with a biplanar dorsal resection wedge, with a minimum evaluation of 48 months of postoperative evolution. In three patients the osteotomy was bilateral, totalling 12 cuffs analyzed. The indication for the technique described was deformity and stiffness for over six months, without improvement with the conservative treatment.
