Biopsy revealed cutaneous metastasis from the primary tumor. This manifestation was previously described as a \"clown nose,\" given their appearance and location. Skin lesions should raise suspicion of malignancy, despite their location at uncommon places, particularly in patients with previous diagnosed cancer.
