Nie","doi":"10.3760/CMA.J.CN101070-20190704-00596","DOIUrl":null,"url":null,"abstract":"Objective \nTo explore the incidence, diagnosis and treatment of retinopathy of prematurity(ROP) in preterm infants born in the Guangdong Women and Children′s Hospital and transported from other hospital. \n \n \nMethod \ns Clinical data of 755 premature infants with ROP at Neonatal Intensive Care Unit, Guangdong Women and Children′s Hospital from January 2013 to December 2015 were retrospectively analyzed.There were 239 cases born in the hospital and 516 cases transported from other hospitals.Their gestational age, birth weight, gender, severity of ROP lesion and clinical data were collected and compared. \n \n \nResult \ns The birth weight in the group of transported from other hospital was lower than that in the group of born in the hospital[(1 290.64±392.87) g vs.(1 586.21±512.74) g], and the difference was statistically significant(P<0.001). The ROP diagnosis of gestational age in the group of transported from other hospital was higher than that in the group of born in the hospital[(35.53±2.81)weeks vs.(34.51±2.17)weeks], and the difference was statistically significant(P<0.001). On the proportion of severe condition [such as lesion area Ⅰ, aggressive posterior retinopathy of prematurity(AP-ROP) and plus combined lesions], in the group of transported from other hospital was higher than that in the group of born in the hospital, and the differences was statistically significant(P<0.001). 摘要

目的探讨广东省妇女儿童医院出生外院转早产儿视网膜病变(ROP)的发病率、诊断及治疗情况。方法回顾性分析2013年1月至2015年12月广东省妇幼医院新生儿重症监护病房755例ROP早产儿的临床资料。239例在医院出生,516例从其他医院转移。收集她们的胎龄、出生体重、性别、ROP病变严重程度及临床资料进行比较。结果外院转运组出生体重低于本院出生组[(1 290.64±392.87)g∶(1 586.21±512.74)g],差异有统计学意义(P<0.001)。外院转移组ROP诊断胎龄高于本院出生组[(35.53±2.81)周vs(34.51±2.17)周],差异有统计学意义(P<0.001)。在病变面积Ⅰ、早产儿侵袭性后视网膜病变(AP-ROP)及合并病变等重症比例上,外院转院组高于本院出生组,差异有统计学意义(P<0.001)。在激光光凝、玻璃体注射、两种手术联合及辅助激光治疗的比例比较中,外院转运组高于本院出生组[60.1%(310/516例)vs.20.9%(50/239例);10.9%(56/516) vs.2.5%(6/239);8.1%(42/516) vs.1.7%(4/239);4.5%(23/516例)vs.1.3%(3/239例),差异均有统计学意义(P<0.001)。结论外迁早产儿ROP出生体重低,ROP病变严重,手术干预率高。提高基层医院ROP筛查水平,及时诊断,高效转运,有助于有效预防早产儿ROP恶化,提高其生存质量。关键词:早产儿视网膜病变;婴儿早产;运输
