Methodology: This observational analytic study was conducted in Outpatient Pediatric Hematology Oncology Dr. Soetomo Hospital. The inclusion criteria were Acute Lymphoblastic Leukemia (ALL) children aged 4-18 years that had undergone chemotherapy treatment, received a cumulative dose of vincristine >12 mg/m2, had no muscle weakness and signed informed consent. The TNS-PV instrument were used to assess VIPN and nerve conduction studies (NCS) was used as the gold standard. Results: A total of 54 children were enrolled, two refused to sign the informed consent. About 60% were male children, 76.9% aged <10 years old, and 94% were ALL-L1. The TNS-PV had an area under the curve (AUC) of 0.638 (95% CI 0.531-0.834) with cut off value of 3.5. It had a sensitivity 87.9%; specificity 42.1%, positive predictive value 72.5%; negative predictive value (NPV) 66.6% and OR (Odds ratio) 5,273. 摘要
目的:分析小儿长春新碱全神经病变量表(TNS-PV)评分作为长春新碱诱导的周围神经病变(VIPN)的诊断工具。方法:本观察性分析研究在Soetomo医生医院儿科血液肿瘤科门诊进行。纳入标准是4-18岁的急性淋巴细胞白血病(ALL)儿童,接受化疗,长春新碱累积剂量bbb12mg /m2,无肌肉无力并签署知情同意。采用TNS-PV仪评估VIPN,以神经传导研究(NCS)为金标准。结果:共有54名儿童入组,其中2名拒绝签署知情同意书。约60%为男童,76.9%年龄<10岁,94%为ALL-L1。TNS-PV曲线下面积(AUC)为0.638 (95% CI 0.531 ~ 0.834),截断值为3.5。灵敏度为87.9%;特异性42.1%,阳性预测值72.5%;阴性预测值(NPV) 66.6%, OR(优势比)5273。结论:TNS-PV评分不能代替NCS作为金标准,但TNS-PV可作为诊断VIPN的替代工具。
