His dental, medical and family history was not relevant. On Intraoral examination, an erupted supernumerary tooth was found lingual to first and second right permanent mandibular premolars. It was irritating the tongue as told by the patient, hence was decided to extract it. After extraction, the root of the supernumerary tooth was found to be dilacerated.
