This case report describes the diagnosis and endodontic management of a three-rooted left maxillary first molar which had three roots and seven canals (MB1, MB2, MB3, DB1, DB2, MP, and DP). This unusual morphology of the maxillary first molar was confirmed using a cone-beam computed tomography scan.
