Angiography<span> identified a giant aneurysm in the ophthalmic<span> segment of the left internal carotid artery. Before seeking care at our institution, the patient had two interventional procedures, both unsuccessful due to difficulties in navigating the microcatheter past the aneurysm neck.</span></span></span><span>4</span>, <span>5</span><span> In our management, after multiple unsuccessful anterograde attempts, we employed a retrograde strategy via the vertebral-basilar-posterior communicating artery route.
