大脑半球间对侧经胼胝体入路治疗巨大丘脑海绵状畸形:二维手术视频。
丘脑海绵畸形的位置较深,且靠近神经血管结构,因此手术难度很大。该区域有多种手术方法,病变位置的细微差别决定了不同的手术路径。对于丘脑上内侧表面的海绵畸形,大脑半球间经胼胝体入路提供了极佳的手术路径,因为丘脑上内侧前三分之二构成了侧脑室体底的外侧。此外,当海绵畸形向外侧延伸时,半球间跨胼胝体入路的对侧辅助入路可提供病变外侧的最佳视野。在这段视频中,我们介绍了一例 31 岁女性的病例,她因巨大出血性丘脑海绵畸形就诊于我院,导致神经功能迅速衰退,表现为共济失调、右侧偏瘫和复视。采用大脑半球间对侧经胼胝体入路切除了海绵畸形。随访3个月时,患者术前的功能障碍几乎完全消除,但右手仍有轻度残余无力。3 年随访时,磁共振成像显示没有活动性海绵畸形,患者的症状也没有复发。本手稿的撰写符合我院机构审查委员会的伦理标准。患者对手术和录像知情同意。
