The patient presented with progressive left lower extremity swelling and a reducible inguinal mass. Imaging suggested a cystic lesion in the groin, prompting surgical exploration. Pathological findings revealed mesothelioma involving regional lymph nodes. With this case, we aim to highlight an exceedingly rare extra-thoracic presentation and underscore the importance of maintaining a broad differential diagnosis for groin masses. Abstract
