Recanalization of a Chronic Total Occlusion in a Coronary Artery with Prior Coil Embolization.

Abstract

Percutaneous coronary intervention (PCI) for chronic total occlusions (CTOs) is particularly challenging in vessels with prior coil embolization as coils alter coronary anatomy and increase procedural complexity. Here, we describe a patient who underwent repeat CTO PCI after the initial procedure was complicated by an Ellis type 3 perforation that required treatment with autologous fat and coil embolization. Recanalization was achieved by traversing the coils deployed in the first procedure. This report underscores the feasibility of recanalizing previously coiled vessels and the importance of applying tailored strategies to optimize clinical outcomes in such complex cases.
