Abstract

Background: Early-stage glottic cancer with anterior commissure involvement presents challenges in exposure and treatment, affecting tumor recurrence and surgical outcomes.

Objective: This study investigated the feasibility of transoral 980 nm and 1470 nm dual-wavelength fiber laser combined with flexible endoscopes for porcine anterior commissure resection.

Material and methods: Five healthy adult pigs were used as experimental subjects. The procedure involved exposing the anterior commissure using a flxible laryngoscope and resecting it using the dual-wavelength laser. The assessment focused on the ease of exposure and resection.

Results: In all five presented pigs, the vocal folds' anterior commissure was well exposed. Furthermore, the anterior commissure of the pigs could be smoothly resected, with the resection range reaching the thyroid cartilage plate, and there were no difficulties in resection due to obstruction by the laryngoscope. No intraoperative or postoperative complications occurred.

Conclusions and significance: The study demonstrates the feasibility and safety of using a flxible laryngoscope combined with a dual-wavelength laser for porcine anterior commissure resection. This approach offers potential as a method for laser surgery in human laryngeal diseases, particularly glottic laryngeal cancer involving the anterior commissure.
