Complaints after testicular cancer metastasis are diverse and vague. Consequently, it is important for clinicians to be familiar with various forms of presentation seen in testicular cancers. Here, we report a case of a 44-year-old patient who presented to the hospital with complaints of abdominal pain, testicular swelling, and bilateral lower extremity edema. This patient was subsequently diagnosed with testicular seminoma and severe bilateral hydronephrosis. Oncology and Urology were consulted for further management. Testicular germ cell tumors are very common among young men. Most cases are diagnosed at early stages.
