A double carcinoma, such as esophageal cancer concomitant gastric cancer is not rare, but a carcinoma of the gastric tube which was substituted for the esophagus is rare. Recently, the incidence of carcinoma of the gastric tube is increasing due to the increasingly long-term survival rate of patients who had esophageal carcinoma. 食管癌次全切除术后11年胃管发生胃癌1例。
本文报告1例食管切除术后重建胃管发生腺癌。患者为66岁男性,11年前因食管癌行食管癌次全切除术。后续上消化道检查显示胃管下部溃疡病变,内镜活检显示腺癌。为了限制癌的侵袭,我们切除胃下管并切开正中胸骨。病理检查显示低分化腺癌伴印戒细胞浸润固有肌层。术后过程顺利,术后6个月无复发。双重癌,如食管癌合并胃癌并不罕见,但胃管癌代替食道是罕见的。近年来,由于食管癌患者的长期生存率越来越高,胃管癌的发病率也在增加。为了确保早期发现第二癌,以尽量减少治疗性切除的损害,应尽可能地进行随访检查。
