卡马西平不良反应与HLA-B*1502基因多态性及血药浓度关系的研究
目的:研究卡马西平(CBZ)的基因多态性、血药浓度与不良反应的关系,以促进临床安全和合理使用。方法:收集上海交通大学医学院附属瑞金医院2017~2019年住院患者的临床医学资料回顾性地。收集HLA-B*1502基因检测及血清CBZ浓度资料,分析HLA-B*15022基因型与患者CBZ不良反应的关系。结果:共有148例患者被纳入研究,其中101例患者接受了HLA-B*1502基因型检测,其中34例基因型阳性,占33.66%。148例患者中,78例接受了CBZ治疗(53.79%),其中60例(86例次)接受了CBZ-血清药物浓度检测在推荐浓度范围内,占检出病例次的61.63%,32例次(37.21%)血清药物浓度低于推荐数据。12例(15.38%)出现CBZ相关的不良反应,其中1例HLA-B*1502纯合子突变患者服用CBZ后出现严重的皮肤不良反应浓度检测对提高CBZ临床应用的安全性具有重要意义。
