We present the case of a 58-year-old male patient with refractory heart failure secondary to residual postinfarction interventricular septal defect who received heart from a 16-year-old asymptomatic, SARS-CoV-2-positive donor. The procedure was performed without complications, with favorable postoperative evolution, discharged 15 days posttransplant. Currently asymptomatic and with no evidence of SARS-CoV-2 infection.
