Fault tree analysis (FTA) is a method that directly focuses on the modes of failure. This paper presents FTA for chance of fire accident due to leakage of liquefied petroleum gas (LPG) in storage yard of catering service. Correlation and regression analysis were used to get the lines of regression in order to place it in normal distribution. Abstract

Hazard evaluation plays a decisive role in the catering service. In order to intensify the safety of the catering service, a hazard assessment and its evaluation is conducted in educational institution. Safety audit is a formal procedure for assessment of the accident potential and expected safety performance for any service or system. Safety audit is being performed with appropriate checklists including occupational hygiene, worker safety, storage and handling and general working conditions.
