We here present a case report of repair of large P-MAIVF that was successfully managed. Postoperatively patient had an uneventful course and patient was asymptomatic at 3rd month follow-up. The cause of P-MAIVF could be an atheromatous ulcer at interleaflet trigone with severely calcified aortic valve as in our case. Abstract
