The effects of ceiling display and natural sounds on stress and anxiety among cardiac patients: A randomized controlled trial

Stress and anxiety are among the most important risk factors of cardiovascular disease. Objectives: The aim of this study was to assess the effects of ceiling display and natural sounds on stress and anxiety among cardiac patients. Methods: This randomized controlled trial was conducted in 2018–2019 in the coronary care units (CCUs) of two teaching hospitals affiliated to Kerman University of Medical Sciences, Kerman, Iran. Participants were 220 cardiac patients randomly allocated through block randomization to four 55-person groups, namely ceiling display group, natural sounds group, combined ceiling display and natural sounds (display sound) group, and control group. The Spielberger State-Trait Anxiety Inventory and the Perceived Stress Scale were used for data collection at the beginning of the study and at the time of discharge from CCU. Data were analyzed by the Chi-square and paired-sample t-tests and the analysis of variance. Results: The mean scores of state and trait anxiety and stress significantly decreased in all intervention groups (P < 0.05) and did not significantly change in the control group (P > 0.05). There were significant differences among the groups respecting the posttest mean scores of state and trait anxiety and stress (P < 0.05). The posttest mean scores of state and trait anxiety in the combined display-sound group and the posttest mean scores of stress in the natural sounds group were significantly less than other groups. 背景:心血管疾病是世界上最常见的慢性疾病,也是导致死亡的主要原因。压力和焦虑是心血管疾病最重要的危险因素。目的:本研究的目的是评估天花板显示和自然声音对心脏病患者压力和焦虑的影响。方法:本随机对照试验于2018-2019年在伊朗克尔曼医科大学附属两所教学医院的冠状动脉监护室(CCUs)进行。研究对象为220例心脏病患者,采用块随机法随机分为天花板显示组、自然声音组、天花板显示与自然声音(显示声音)相结合组和对照组,每组55人。在研究开始时和从CCU出院时使用Spielberger状态-特质焦虑量表和感知压力量表进行数据收集。数据分析采用卡方检验、配对样本t检验和方差分析。结果:各干预组状态焦虑、特质焦虑、应激平均得分均显著降低(P < 0.05),对照组无显著变化(P < 0.05)。状态焦虑、特质焦虑、应激后测均分组间差异有统计学意义(P < 0.05)。综合播放声音组状态焦虑和特质焦虑的后测平均分以及自然声音组的应激后测平均分均显著低于其他各组。结论:吊顶显示和自然声音能显著降低CCU心内科患者的状态和特质焦虑和应激,且两者结合效果更为显著。
