Effect of Early Continuous Veno-Venous Haemofiltration in Severe Acute Pancreatitis for the Prevention of Local Pancreatic Complications

Results In both groups, there was a decrease in amylase, lipase, CRP, IL-6, IL-10, TNF-alpha, Ranson score, Balthazar score, and APACHE-II score after 72 hours, but the decrease was significantly greater in CVVH patients. There were no any local pancreatic complications in CVVH patients, but 1 patient had an acute peripancreatic fluid collection, 2 patients had pseudocyst, and 2 patients had walled-off necrosis (WON), and a mortality one was seen in the conventional treatment group. 重症急性胰腺炎早期持续静脉-静脉血液滤过对预防局部胰腺并发症的作用
目的比较常规治疗和连续静脉-静脉血液滤过(CVVH)治疗重症急性胰腺炎(SAP)预防假性囊肿和坏死的疗效。患者和方法。42例患者分为两组:常规治疗组24例,CVVH组18例。常规治疗组患者根据症状和病因进行治疗。CVVH组患者接受症状治疗,并在入院后2小时内进行CVVH。结果72小时后,两组患者的淀粉酶、脂肪酶、CRP、IL-6、IL-10、TNF-α、Ranson评分、Balthazar评分和APACHE-II评分均下降,但CVVH患者的下降幅度更大。CVVH患者没有任何胰腺局部并发症,但1例患者出现急性胰周积液,2例患者出现假性囊肿,2例出现壁坏死(WON),常规治疗组出现1例死亡。结论早期CVVH可预防SAP患者假性囊肿的形成,有利于SAP患者的生存。
