目的探讨经直肠超声引导下前列腺活检结合MRI隔室定位对前列腺癌的诊断价值。方法选择前列腺特异性抗原水平升高的患者50例。所有患者活检前均行MRI扫描,并将前列腺划分为12个区室。分裂的方向和区域与超声一致。对12个样本进行了系统的针活检,并对可疑的隔室进行了额外的活检。根据组织病理学,分析比较系统12例穿刺活检与系统活检合并疑似隔室穿刺的阳性检出率。结果系统活检联合可疑间室穿刺前列腺癌检出率显著高于系统12例穿刺前列腺癌检出率(42.0% vs 36.0%,P < 0.05)。系统活检合并疑似隔室穿刺导致血尿5例,血便13例,发热1例,无其他并发症。结论经直肠超声联合MRI隔室定位可提高前列腺活检的预后,有利于前列腺癌的早期诊断。
