If combined with another migrated fragment and happened in a very aged patient, to the best of our knowledge, it was never reported in the literature. We reported a case of 85-year-old female patient who suffered from severe low back pain and right sciatica after lumbar sprain for 1 month and aggravated for 5 days. Magnetic resonance imaging demonstrated a long longitudinal tumor-like lesion, extending from L2-3 to L3-4 anterior epidural space and occupying about anterior one third of the vertebral canal. Besides, there was another tumor like lesion located at the right L3-4 lateral foramen.
