Surgical resection in the form of radical nephrectomy and caval thrombectomy represents the only option to obtain local control of the disease and is associated with durable oncologic control. Herein we present a case of 82 year old male presenting with haematuria and right flank pain with palpable mass in right lumbar region. CT scan revealed presence of right upper pole renal mass limited within Gerota’s fascia with thrombus in right renal vein extending to IVC. Patient underwent laparoscopic right radical nephrectomy with cavatomy for thrombus extension.
