Abstract

If Pharmaceutical Management and Counseling Services are required for active participants in the health care industry, then innovative strategies must be adopted to accurately document pharmacist's contributions for the area. We therefore attempted to develop a new documentation method to enable pharmacy departments to achieve certain patient outcomes.
