Endoscopic adenoidectomy with microdebrider

Abstract

The classic method of adenoidectomy with different curettes is used worldwide. Among the recent methods of adenoidectomy, we will show the microdebrider combined with transnasal video-endoscopic view. The purpose is to demonstrate and describe the adenoidectomy using the microdebrider combined with endoscopic view, emphasizing the advantages and disadvantages compared with traditional methods.

We have studied 50 patients diagnosed with obstructive hypertrophy of pharyngeal tonsils.

It was possible to make the adenoidectomy with microdebrider by direct view in all of the cases. The advantages are as follows: (a) realization of the surgery by direct view; (b) a better view of the peritubarian recesses decreasing the possibility of lesion; (c) better control of the adenoidal tissue to be removed, mainly in the recesses; (d) better and direct control of the bleeding; (e) avoidance of temporary gauze blockage; and (f) recording of the procedure on videotape. Among the disadvantages, we can consider the following: (a) bleeding sometimes makes the endoscopic view difficult, which may prolong the surgical time; and (b) the cost of the involved equipment. In spite of the advantages and disadvantages of this method, the authors believe in its utility as one more valid option for the surgeon.
