摘要

直接腹膜复苏(DPR)已被发现是一个有用的辅助管理危重创伤患者。DPR是在损伤控制手术后进行的,通过在肠系膜中留下手术引流管,暂时关闭腹部,术后通过手术引流管输送腹膜透析液,并通过临时关闭清除。在原始动物模型中,发现腹腔透析液输注可增加内脏微循环血流量,减少失血性休克后发生的缺血性损伤。研究还发现,DPR可以最大限度地减少继发于休克的异常免疫反应,并有助于多系统器官功能障碍。在随后的人体试验中,实施DPR在几个关键类别中产生了显著影响。接受DPR的创伤性损伤患者实现最终筋膜闭合的时间明显缩短,实现初级筋膜闭合的可能性更高,腹部并发症也更少。DPR的使用已进一步扩大,作为紧急普通外科病人和人类尸体器官捐赠者的移植前护理的有用辅助手段。
