摘要

本文报告一例88岁女性,因中心静脉导管相关的双侧胸腔积液,左侧胸腔积液,右侧胸腔积液引流。引流可防止右侧胸膜腔积液,提示没有输液外渗,两胸膜腔之间也没有连接。本例双侧胸水的唯一解释是淋巴连接。虽然中心静脉导管损伤血管可引起导管相关性胸水,但中心静脉导管尖端位于右侧锁骨下颈静脉汇合处或上腔静脉的淋巴管开口内最有可能引起导管相关性胸水。心包积液也可由逆行淋巴流通过肺淋巴链引起。
