Methods: We describe here a patient with this condition, presented with large perianal tumor, with a long-standing history of perianal endometriosis in episiotomy scar. This was initially evaluated with a Magnetic Resonance Imaging (MRI) and the diagnosis was confirmed by biopsy. Result: The evolution of more wide excision surgery resulted in an increase in large defects of the perineal and perianal region.
