A Systematic Review of Cognitive Orientation to Daily Occupational Performance for Stroke
This study aimed to provide information on the evidence level of studies, participant characteristics, self-selected goals, evaluation areas and tools, and intervention effects through a systematic review of studies regarding CO-OP (cognitive orientation to daily occupational performance) for stroke. The articles selected for this study were published between January 2015 and December 2022, and the keywords used for the search were “Cognitive orientation to daily Occupational Performance,” “Stroke,” and “CO-OP.” The databases searched were Science Direct, Academic Search Complete, Korean Studies Information Service System (KISS), and Research Information Sharing Service (RISS). The searched articles were screened through the PRISMA process, and 10 articles were finally selected for analyzing. The evidence level of the studies was high, and a randomized controlled trial (RCT) design was used in 50% of the studies. The most common cause of stroke is ischemia, and the common criteria for selecting participants are cognitive ability and ADL. Regarding the self-selected goals, dressing, personal hygiene, grooming, and functional mobility were preferred in the ADL area, whereas meal preparation, clean up, and communication management were preferred in the IADL area. Evaluation of occupation area was the most common, and the most frequently used evaluation tools were the COPM and PQRS. Regarding the effects of CO-OP interventions, most studies have shown that the quality and satisfaction of occupational performance have improved. The most common number of sessions and treatment duration was 12 sessions (60 min per session). This systematic review shows that CO-OP is an effective intervention for stroke and provides basic information on its clinical application for therapists and ideas for future research.
