We present a case with rare complications, unrelated to inadequate orthopedic treatment. In addition, the study is completed by conducting a review of the literature with the different systems used to classify this type of injury. A 49-year-old male athlete with a lateral ankle\nsprain while practicing basketball who presented significant edema, which progressed to anterolateral skin necrosis in 2 weeks. After scarectomy, he presented a deep wound with exposure of the capsulo-ligamentous structures, turning the injury into an open one. He associated a complete injury of the superficial peroneal nerve.
