Lateral ankle sprain is a frequent injury that heals quickly with correct treatment. We present a case with rare complications, unrelated to inadequate orthopedic treatment. In addition, the study is completed by conducting a review of the literature with the different systems used to classify this type of injury. A 49-year-old male athlete with a lateral ankle sprain while practicing basketball who presented significant edema, which progressed to anterolateral skin necrosis in 2 weeks. After scarectomy, he presented a deep wound with exposure of the capsulo-ligamentous structures, turning the injury into an open one. He associated a complete injury of the superficial peroneal nerve. Outpatient cures were startedwith associated negative pressure therapy, healing the wound after 4 months of evolution, returning to sports practice at 8 months.
