This case report aims to further discuss the importances and concerns regarding the anesthesia management of decompression craniectomy in morbidly obese patients. Case presentation: A 46-years-old male patient was diagnosed with intracerebral hemorrhage on regio left external capsule due to hypertensive emergency suspected from Middle Cerebral Artery (MCA) aneurysma rupture. Decompression craniectomy with clot evacuation was planned. This patient has morbid obesity. Prediction of difficult intubation and mask ventilation can be assessed using LEMONS, MOANS, Mallampati score and STOP-Bang Questionnaire. The dosage of anesthetic agents used in these patients had been adjusted according to current recommendations. Intraoperatively, patient’s hemodynamic was successfully maintained stable. 摘要

背景:接受任何神经外科手术的病态肥胖患者的麻醉管理尤其具有挑战性。本病例报告旨在进一步讨论病态肥胖患者颅骨减压术麻醉管理的重要性和关注点。 病例介绍:一名 46 岁的男性患者被诊断为左侧外囊区域脑出血,原因是高血压急症,疑似大脑中动脉(MCA)动脉瘤破裂。计划进行减压开颅手术并清除血块。该患者患有病态肥胖。插管和面罩通气困难的预测可使用 LEMONS、MOANS、Mallampati 评分和 STOP-Bang 问卷进行评估。这些患者使用的麻醉剂剂量已根据当前建议进行了调整。术中,患者的血流动力学成功保持稳定。术后,重症监护室对患者进行了密切监测。 结论对于接受神经外科手术的病态肥胖患者来说,必须充分了解如何在术前、术中和术后选择有效的麻醉管理方法,以获得良好的治疗效果。
