This study used a descriptive research design by reviewing medical record documents and FGDs. Document review was conducted on 111 medical records taken by random sampling. Identification of the root of the problem is carried out through FGD activities by applying the 5 Why's. The results of the FGD showed that there were three factors that contributed to the incompleteness of medical records at X Hospital, namely humans, machines, and methods. 摘要

病历的完整性是衡量医院医疗服务质量的一个指标。X 医院的病历完整性仍未达到国家规定的标准。本研究的目的是找出马兰市 X 医院门诊病历文件不完整的根本原因。本研究采用描述性研究设计,通过审查病历文件和专题小组讨论进行研究。通过随机抽样对 111 份医疗记录进行了文件审查。通过 "5 个为什么 "的 FGD 活动确定了问题的根源。FGD 的结果显示,导致 X 医院病历不完整的因素有三个,即人、机器和方法。发现问题的根源在于没有电子病历、病人数量与医生服务时间分配不成正比、医疗和护理评估表合二为一、医生助理缺乏知识、质量指标和远程医疗 SOP 仍未与门诊区分开来。
