Diagnosis is difficult because of the lack of specific signs or symptoms. <b>Case:</b> A 62-year-old man underwent endoscopic retrograde cholangiopancreatography for choledocholithiasis. A cardiovascular collapse occurred during the procedure. The echocardiography showed air within the right ventricle.
