增强现实(Augmented reality,AR)有望成为神经外科的术中辅助技术。1 神经胶质瘤的清醒开颅手术(AC)有利于切除范围、存活率和术后神经功能的恢复。一名 29 岁的右撇子女性在一次全身抽搐后到当地医院就诊。头部核磁共振成像显示左侧额叶中回有一个广泛扩散的肿瘤。在患者熟睡状态下进行左额叶开颅手术后,AR 被用来指示肿瘤与皮层下纤维的边界,包括皮质脊髓束、前枕下束和扣带回束。我们在 AR 的辅助下切除了额中回表面的肿瘤。在对额叶斜束和枕下束进行皮层下刺激(SCS)后,患者在图片命名测试中停止了对物体的命名,而对左侧扣带回的SCS刺激则使患者在Stroop测试中弄错了颜色。AR 鉴定出的皮层下纤维与 SCS 引起症状的部位相吻合。我们最终切除了大部分肿瘤。术后核磁共振成像证实切除率为 96.2%。患者出院时没有出现任何新的神经功能障碍。AC 配合 AR 可用于优势半球胶质瘤的切除。患者同意接受手术并同意公布其影像。我们医院的伦理委员会不要求病例报告获得批准。
