It is a rare complication of <i>Escherichia coli</i> septicemia. We herein report a case of left eye endogenous endophthalmitis with uncontrolled type 2 diabetes mellitus with pyelonephritis associated with <i>Escherichia coli</i> septicemia. Vitrectomy was done along with intravitreal antibiotics and steroids.
