The patient underwent an incisional biopsy of the lesions, which showed a CLL with areas of large B-cell lymphoma, consistent with Richter transformation. Following medical and radiation-based therapy of these lesions, the patient returned 3 months later with inferomedial preseptal swelling in the contralateral eye, which biopsy proved to be recurrent/resistant low-grade CLL with a posttreatment extranodal marginal zone B-cell lymphoma pattern. 摘要
一名 74 岁的男性患者曾患慢性淋巴细胞白血病（CLL），其操作系统的下穹窿和颞上结膜出现大面积鲑鱼色斑片状病变。患者接受了病变切口活检，结果显示其为伴有大 B 细胞淋巴瘤区域的 CLL，符合里氏转化。在对这些病变进行药物和放射治疗后，患者于3个月后复发，对侧眼球内侧隐窝前肿胀，活检证实为复发/耐药的低级别CLL，并伴有治疗后的结节外边缘区B细胞淋巴瘤形态。本病例是一种罕见的里氏转化CLL表现，也是治疗后复发/耐药的眼眶CLL，具有边缘区B细胞淋巴瘤样形态。
