Following 3 months of preoperative orthodontic treatment, in October 2018 under general anesthesia the patient underwent a right mandibular condylectomy, Le Fort I osteotomy, right mandibular sagittal split ramus osteotomy, and left mandibular inverted L ramus osteotomy. In examinations up to 3 years after surgery, good results were noted. For this case of severe facial asymmetry with mandibular condyle hyperplasia, early surgery and condylectomy were performed simultaneously to significantly shorten the total treatment time. 摘要

对于单侧下颌髁状突增生的患者,是同时进行髁状突切除术和正颌外科手术,还是分两个阶段进行正颌外科手术,目前仍存在争议。本文报告了一例面部不对称伴下颌骨髁状突增生的病例,该病例同时进行了髁状突切除术和正颌外科手术。一名 28 岁的女性因主诉下颌骨左偏和右侧颞下颌关节(TMJ)噪声而到我科就诊。经过多次影像学检查,诊断为面部不对称,右下颌骨髁状突增生。经过 3 个月的术前正畸治疗后,2018 年 10 月,患者在全身麻醉下接受了右下颌髁突切除术、勒堡 I 型截骨术、右下颌矢状劈裂臼齿截骨术和左下颌倒 L 型臼齿截骨术。术后 3 年的检查结果显示,手术效果良好。对于这个面部严重不对称并伴有下颌骨髁状突增生的病例,早期手术和髁状突切除术同时进行,大大缩短了总的治疗时间。术后未发现颞下颌关节异常或后翻,证实了早期手术方法的有效性。
