He suffered from recurrent pneumothorax within half a year and experienced a relapse after receiving conservative treatments.</p><p><strong>Case presentation: </strong>Diagnostic workup revealed a cystic lesion in the right middle lobe, which has been interpreted as a bulla during the initial chest CT scan. Due to recurrent pneumothorax and poor response to the conservative treatments, the patient underwent bullectomy and pleurodesis.
