He was diagnosed with hemophilia A at the age of three years old, and was also infected with hepatitis C and human immunodeficiency virus (HIV). He complained of chest pain at home and was brought to our institute and diagnosed with acute myocardial infarction. Concerned about the risk of bleeding, coronary artery bypass grafting was selected to avoid permanent administration of antiplatelet agents. Although there were concerns about bleeding caused by various coagulation factor abnormalities due to the use of cardiopulmonary bypass, we could accurately assess the coagulation factors by using thromboelastography and replenishing them in appropriate amounts. 我们报告了一例使用血栓弹性成像技术为 42 岁男性血友病 A 患者进行冠状动脉旁路移植术的病例。他在家中主诉胸痛,被送到我院后被诊断为急性心肌梗死。考虑到出血的风险,我们选择了冠状动脉旁路移植术,以避免长期服用抗血小板药物。虽然担心因使用心肺搭桥术而导致各种凝血因子异常引起出血,但我们可以通过血栓弹力图准确评估凝血因子,并适量补充凝血因子。手术照常安全进行,顺利完成。
