An algorithm has been formulated for the management of CAIS starting in adolescence, highlighting the conservative approach for those patients unwilling to undergo gonadectomy. <i>Conclusions:</i> Primary amenorrhea and delay in puberty development may provide clues, ultimately leading to a diagnosis of CAIS.
