This paper evaluates the economic effect of family physician contracting by employing microdata of patients with chronic diseases at the Yuyuantan Community Health Service Center, Haidian District, Beijing, from January 2019 to May 2021. This research contributes to the theoretical discourse in healthcare services by introducing a modified Anderson model. The empirical findings demonstrated that family physician contracting effectively reduced healthcare costs for patients with chronic diseases. Notably, the effect of family physician contracting on healthcare costs was more pronounced among patients with chronic diseases aged less than 65 years. At the same time, it is inferred that expediting the reform of the health insurance payment system, coupled with the promotion of family physician contracting services, would enhance the wellbeing of patients with chronic diseases. 摘要

随着人口老龄化的加剧,与慢性病相关的经济负担不断加重。在各种管理策略中,家庭医生签约服务作为一项旨在应对这一挑战的重要举措脱颖而出,并于2016年在中国正式推行。然而,目前的研究对发展中国家家庭医生签约服务的经济效益关注相对较少。本文采用北京市海淀区玉渊潭社区卫生服务中心 2019 年 1 月至 2021 年 5 月慢性病患者的微观数据,评估了家庭医生签约服务的经济效应。该研究通过引入修正的安德森模型,为医疗服务领域的理论研究做出了贡献。实证研究结果表明,家庭医生签约服务有效降低了慢性病患者的医疗费用。值得注意的是,家庭医生签约服务对医疗费用的影响在 65 岁以下的慢性病患者中更为明显。同时,可以推断,加快医保支付制度改革,同时推广家庭医生签约服务,将提高慢性病患者的福利。这项研究为发展中国家家庭医生签约服务的当前和未来发展提供了宝贵的见解。
